Summary:

The Care Coordinator provides person-centered coordination and navigation services to individuals and families accessing Brightwater Health. The Care Coordinator supports individuals from initial contact through connection to appropriate services and ongoing coordination of care.

Responsibilities may include responding to initial requests for services, gathering information needed to facilitate access, identifying needs and barriers, coordinating internal and external referrals, connecting individuals with Brightwater programs and community resources, facilitating warm handoffs, and providing ongoing care coordination.

The Care Coordinator works collaboratively with individuals, families, clinical staff, Brightwater Health programs, healthcare providers, community organizations, and other systems to promote timely access to services and continuity of care.


Duties:

  • Serve as a welcoming point of contact for individuals seeking Brightwater services.
  • Gather information necessary to facilitate appropriate access to services.
  • Complete required initial contact, demographic, insurance, consent/release, and referral information.
  • Identify immediate needs, barriers to care, and potential care coordination needs.
  • Provide information regarding Brightwater Health services and community resources.
  • Coordinate internal referrals and connections among Brightwater Health programs.
  • Coordinate external referrals and connections with community providers and organizations.
  • Facilitate warm handoffs between individuals and appropriate services.
  • Assist individuals in navigating behavioral health, health care, housing, transportation, financial, employment, education, and other systems.
  • Follow up on referrals and service connections to determine whether the individual successfully accessed the identified resource.
  • Coordinate communication among providers and systems when appropriate releases/consents are in place.
  • Identify barriers that interfere with engagement and work collaboratively to address those barriers.
  • Support transitions between services, programs, levels of care, and community systems.
  • Maintain accurate and timely documentation.
  • Participate in community outreach and relationship-building activities.
  • Participate in team-based rotation for intake/access coverage as assigned. 

Minimum Qualifications:

  • Bachelor's degree in human services or related field and 2,000 hours of mental health experience.
    OR high school diploma and 6,000 hours of mental health experience.
  • Valid Driver's License and reliable vehicle with proof of insurance.
  • Ability to submit and pass a DMV/Criminal background check.
  • Strong interpersonal and cultural competency skills.
