Summary:

Community Support Providers work with a caseload of children diagnosed with a mental illness to coordinate and provide services to them and their families. This position works to help the child and family to address skills deficits in the following areas: mental health, social skills, educational skills, vocational skills, and recreational skills. This position works closely with other community support providers, clinical staff and other care providers to help the client meet their identified goals. Services are provided in the clinic, community and client’s home.

Duties:

  • The program aims to improve the functioning of children in the home, in school, and in the community.
  • The provider will assist families in accessing necessary services to meet mental health needs of children ages 0-18.
  • Collaborate with clinical team to create, review, and implement treatment plan goals and objectives.
  • Provide individual, group, and family skills training based on the treatment plan in the home, school, and community.
  • Requires flexible scheduling and some evening work.

Minimum Qualifications:

  • Must meet the qualifications of a Mental Health Practitioner in Minnesota Statute 2451.04
    • Bachelor's degree in human services or related field and 2,000 hours of mental health experience.
      • OR high school diploma and 6,000 hours of mental health experience.
  • Valid Driver's License and reliable vehicle with proof of insurance.
  • Ability to submit and pass a DMV/Criminal background check.
  • Strong interpersonal and cultural competency skills.
